Film/Video Policy and Equipment Access Form

Please Read Carefully

  1. Eligibility

    Only students currently enrolled in Film/Video classes are permitted access to Film/Video equipment and facilities (unless otherwise specified). PRO ARTS/COF cross-registered students are not permitted to sign out Film/Video equipment for overnight use. Access is determined by course requirements and does not guarantee availability of all equipment. The film/video cages are closed during the break and over the summer.
  2. Responsibility

    All Film/Video users are financially liable for any loss, damage, or theft of equipment, including accessories, cases, and cables.
  3. Pick-Up and Return

    Equipment must be picked up and returned during scheduled reservation hours. The Video cage operates by reservation only. Back-to-back reservations are not permitted.
  4. Reservation Window

    Reservations are held for 30 minutes only. After that, the reservation will be forfeited. All equipment must be picked up and returned in person.
  5. Identification Requirement

    A valid MassArt ID is required to access Film/Video equipment. No equipment will be checked out without a current, valid MassArt ID.
  6. Check-In Procedure

    Equipment is not considered returned until it has been fully checked in by cage staff. Users are responsible for all damages and missing items until the equipment is verified and accepted.
  7. Late Returns

    Inexcusable lateness in returning equipment will result in loss of sign-out privileges. See the Late Policy section for details.
  8. Equipment Repairs

    Do not attempt to repair any equipment. All issues must be reported immediately to the Studio Manager.
  9. No Equipment Sharing

    Sharing equipment or checking out equipment on behalf of another person is strictly prohibited and will result in a loss of access privileges.
  10. Incomplete Returns

    Equipment returned incomplete (e.g., missing accessories) is considered late and subject to penalties.
  11. Replacement Fees

    Fees will be assessed for missing or damaged cables, accessories, and other items—in addition to the replacement cost.
  12. Insurance Requirement

    Equipment packages valued over $5,000 require proof of insurance.
  13. Renewals

    If available, equipment may be renewed only once.

    If you encounter any issues with the renewal process, please contact the equipment cage.

    Note: The Video Cage does not accept renewals by phone or email.
  14. Electronic Signature Agreement

    By selecting the “I Accept” button, you agree to the terms of this Access Policy and all related Film/Video agreements. Your electronic signature is legally equivalent to your manual signature, and constitutes full acceptance of all terms and conditions.

EQUIPMENT LATE POLICY

1st Offense:

Late by 30 minutes or more = Loss of reservation privileges for one month

2nd Offense:

Loss of checkout privileges for one semester

Severe Lateness (24+ hours):

$100 late fee plus $25 per additional business day

Please Note: There are no exceptions to this policy. Any violation may result in permanent revocation of access to Film/Video facilities and equipment. This policy is subject to change.
